1. Collection of Personal Information

Life Protector Limited may collect personal information from third parties including mortgage lenders and banks, insurance companies, KiwiSaver and investment providers, credit reporting agencies, employers, accountants, solicitors and conveyancers, real estate agents, government agencies, referrers, family members, and publicly available sources.

2. Types of Information Collected

Full name, date of birth, contact details, residential address, employment information, income and financial information, bank account information, credit history and reports, identification documents, visa and residency information, insurance information, property ownership information, and KiwiSaver/investment information.

3. Purpose of Collection

Providing mortgage, loan and insurance advice; assessing eligibility for financial products; preparing applications; meeting legal and regulatory obligations; conducting AML/CFT checks; managing our client relationship; communicating regarding our services.

4. Recipients of Information

Banks and lenders, insurance providers, KiwiSaver and investment providers, credit reporting agencies, government agencies where required by law, professional advisers, technology and software providers, and compliance and audit service providers.

5. Legal Obligations

Collection is required under the Privacy Act 2020, Anti-Money Laundering and Countering Financing of Terrorism Act 2009, Financial Markets Conduct Act 2013, Financial Service Providers legislation, and other applicable NZ laws.

6. Storage and Protection

We take reasonable steps to protect your personal information against loss, unauthorised access, misuse, modification, disclosure, or destruction. Information may be stored electronically, in cloud-based systems, and in secure physical records.

7. Overseas Disclosure

Some service providers may store information outside New Zealand. We take reasonable steps to ensure comparable privacy protection as required under the Privacy Act 2020.

8. Your Rights

You have the right to request access to and correction of your personal information, ask questions about how it is used, and make a privacy complaint if you believe your rights have been breached.

9. Contact

Pursuant to the Privacy Act 2020, the following information is relevant to you.

1. The personal information collected in this form is collected for the purpose of assessing your application for financial products and services and may be given to Product Providers at the Financial Adviser's discretion.
2. The information is being collected to determine your eligibility for the financial products and services you seek.
3. You are aware that the Product Provider will make information available to its insurer, credit reporting agency, Administrator, trustees, assignees, and other authorised parties. The Financial Adviser and Product Provider may also use your information for market research and to notify you of relevant products and services.
4. You have a right to request access to and correction of any personal information held by the Financial Adviser, Product Provider, or credit reporting agencies, subject to the Privacy Act 2020.
5. Where personal information is collected from a third party, we will take reasonable steps to notify you of the collection, purpose, intended recipients, and your rights of access and correction, unless an exception under the Privacy Act 2020 applies.
6. By making this application, I/We understand and authorise that:
   1. The Financial Adviser, Product Provider and Recipients will collect personal information from third parties including credit reporting agencies, banks and employers.
   2. Information will be made available to the Product Provider, its Administrator, insurer, underwriter, trustee, the Adviser, and future assignees.
   3. The Product Provider may disclose personal information to the Financial Adviser during the term of the product to answer queries or assist with financial arrangements as circumstances change.
   4. The Financial Adviser, Product Provider and Recipients may disclose personal information to credit reporting agencies and authorised third parties.
   5. Credit reporting agencies may hold and use personal information to provide credit reporting services.
   6. Credit reporting agencies may provide personal information to their customers using credit reporting services.
   7. The Financial Adviser, Product Provider and Recipients may use credit reporting agency services for future financial products. This includes monitoring services to receive updates if personal information changes.
   8. The Financial Adviser, Product Provider and Recipients may give information to credit reporting agencies about default in payment obligations.
   9. Credit reporting agencies may provide information about payment defaults to other customers.
   10. The Product Provider relies on the information provided; failure to provide or materially incorrect information may result in the application being declined.
7. If accepted, I/We authorise recipients to disclose personal information to an associated Product Provider to open and maintain any relevant financial product.

Customer Declaration

I/We declare, acknowledge and agree that:

1. The information contained in this application is true and correct.
2. Where the Financial Adviser has assisted in completing this form, I/We have authorised this and the information is true and correct.
3. I/We are at least 18 years of age.
4. I/We have not withheld any information on our financial position or commitments.
5. I/We are able to meet legal and valuation costs.
6. We should review personal risk insurance requirements as part of the intended financial advice transaction.
7. Should our circumstances change while any product or service remains in place, we have an obligation to notify the Financial Adviser.
8. The Financial Adviser may receive a commission for writing, placing, or referring any financial product.
9. The Financial Adviser generally does not charge clients for services (unless specifically negotiated), but receives commission from the Product Provider. The Financial Adviser is not an employee, agent, or partner of the Product Provider.
10. If a separate consultancy fee or commission clawback applies, these costs are available on the public website and disclosure documents.
11. If the Financial Adviser has an ongoing commission arrangement, the Product Provider will periodically disclose the relevant balance or value to the Financial Adviser.
